Protox: Customized Botox

Gone (mostly) are the days of the post-injection frozen face, the seemingly emotionless Botox-bot shopping its way down Madison Avenue. As highly trained plastic surgeons in Manhattan’s upper east side beauty mecca, our needlework is getting much more subtle, “smarter”, and more customizable! Over the past year, I have seen a huge increase in younger patients, hoping to incorporate a little Botox into their regular beauty regimen to help prevent the development of fine lines and wrinkles, and to set the stage for lovely, natural, beautiful wizening (I don’t like to say “aging”). My patients, both men and women, want to look natural, just a little more refreshed and less stressed. They want to be able to express themselves with their faces- to laugh, get mad and even cry.

Introducing Protox

Often, as part of their jobs, they must be able to express themselves with their faces to be effective and successful. Lucky for us all, with some experience and knowledge, Botox is highly customizable! Introducing what I’ve dubbed “Protox” – custom Botox designed with your professional goals in mind. A personal brand identity is key to one’s career and the face often says it all.

The Case of The Lawyer

Attorneys must have the ability to look disapproving. This patient always needs to maintain a little corrugator movement (this is the muscle group that give you the vertical “11 lines” between the eyebrows, and furrows the brow) in order to successfully argue a case or negotiate a mediation.

When The Psychiatrist is the Patient

A Psychiatrist needs to maintain a sympathetic demeanor without displaying a disapproving, “frozen” look. In contrast to the lawyer, this patient needs to seriously weaken movement in the corrugators so they can relax and listen to what their patients are saying without looking judgy, but keep a little movement in the frontalis muscle (the muscle that raises the eyebrows) so they can still look like they care about what they’re hearing.

Let The Teacher Teach

He needs to maintain the ability to look surprised and to be sympathetic. This patient needs to retain almost all movement of the frontalis muscle that raises the eyebrows, but can often benefit greatly from Botox used to soften the crow’s feet and raise up the arch of the eyebrow.

The Poker Player

This patient needs Botox to retain her poker face and help eliminate the look of surprise (frontalis muscle) and some of the most common facial “tells” such as clenching the jaw and flexing the masseter muscle at the angle of the jaw. (masseter muscle Botox) It’s important for poker players to be able to keep all emotion under wraps.

The Actor Needs to Emote

Performers need to be able to express all emotions, but must also look permanently airbrushed with subtle touches of Botox in a number of areas.

Botox for Masseter Muscle Hypertrophy

masseter muscle hypertrophy- Botox in NYCMasseter muscle hypertrophy is enlargement of the masseter muscle, and can contribute to a harsh, square look at the angle of the jaw. The masseter muscle is one of the muscles activated during chewing. It’s located at the “angle of the mandible”, or “angle of the jaw”. Botox can be used to relax this muscle, and to soften the look of the lower face. I usually start with 50 units of botox (25 units on each side). When Botox is used to relax the facial muscles that cause fine line, the maximum effect is seen in 7-10 days. When Botox is used to reduce masseter muscle hypertrophy, the maximum effect is not seen for 3 months. Results can last up to 9 months. I recommend repeat injection every 6 months if you would like to maintain your results. Check out what ELLE Magazine has to say about it.

How I Beat My Daily Grind with Botox

Some people eat when they’re stressed. Some shop. Some shop for food that they then eat. Me? I clench my jaw. Aside from my knotted up shoulders, my masseter muscles are the greatest bearer of my bodily stress. If you suffer from teeth grinding,TMJ or any similar jaw-related tension condition, I know you feel my pain and frustration. To make matters worse, most of the clenching was happening at night, so I couldn’t even catch myself in the act and modify the behavior. I would wake up every morning with tightness and soreness along the side of my face that I would try – fruitlessly – to massage away. Before you could say “night guard’, I had cracked the filling on a molar and wore down another molar to a rounded stump. I wanted to find a real preventative solution that would help me avoid extra trips to the dentist – twice a year for my cleaning is more than enough, thank you.

Grinding Your Teeth? Botox to the Rescue

Botox, which is fast becoming a panacea for all sorts of health and beauty issues, is a very useful tool for dealing with jaw tension. Makes sense – Botox paralyzes muscles, and teeth grinding is caused by the masseter muscle going into nocturnal overdrive. Weaken the masseter muscle and you alleviate most, if not all, of this tension.

It’s Quick & Painless

I visited Dr. Dara Liotta for this unbelievably quick and painless procedure. We’re talking ten minutes and two tiny pinches. The first time she did the procedure she used 20 units of Botox on each side. When Botox is used to relax the facial muscles that cause fine lines, the maximum effect is seen in 7-10 days. When Botox is used on the masseter muscle, the maximum effect is not seen for 3 months…and results can last up to 9 months. Dr. Liotta recommended I get repeat injections every 6 months to maintain results. She also said that many people enjoy very long term results after three treatments. Within a month, I noticed a huge difference in the pain and tightness around my jaw. I no longer woke up every morning feeling like I’d been gnawing on a wad of Hubba Bubba all night. Now, nine months since my first injection, I’m happy to say I’ve not had to make a single extra dental visit!

As if the reduction in pain and dental visits was not enough, the procedure has cosmetic benefits as well. I have always thanked my Russian ancestors for my chiseled cheekbones. Since the Botox reduced the size of my Masseter muscle, it also reduced and softened my jawline, creating a more dramatic angle from my cheek to my jaw.This allowed my cheekbones to “pop” even more than before. When Dr. Liotta showed me pictures taken before and then 2 months post-procedure, I was floored.

Maintenance Botox

I have a lot of patients who ask me about the role of what I call “maintenance Botox or Dysport”, when fine lines and wrinkles are just starting, as opposed to “treatment Botox or Dysport”, when the lines and wrinkles are driving you nuts.

A study was published in 2006 by Dr. William Binder in the Archives of Facial Plastic Surgery demonstrating the safety and effectiveness of using BOTOX® over a long period of time to treat and prevent facial wrinkles. This study involved identical twin sisters one of whom received regular BOTOX® over a 13 year period and the other who only used it only a few times in her forehead and glabellar regions (area between the eyebrows) and not at all for her crows feet area. No adverse effects or problems were encountered in either twin but the effects on wrinkle prevention in the treated twin are dramatic particularly in the crows feet area.  Note that the photos of both sisters were taken when neither of them had any Botox on board.  Amazing!
